After upgrading 150+ (with 47more to go) systems in the last 3 weeks, I recommend two steps that seem to make a huge difference.
1. Make sure the account you are logged in with when you run the upgrade is an ADMIN account. 2. Download and use the "Media Creation Tool.exe" to do the upgrade. Download it at https://www.microsoft.com/en-us/software-download/windows10/
I stopped caring why a long long time ago, but for whatever reason step 2 is HUGE! Ignore all the other prompts and temptations...pretend you don't see them...download and run (and respond to) the Media Creation Tool and you may still have problems, but it's much much likely to work.
As may have already been mentioned, a reboot, sometimes two, never hurts.

Oh, and I might mention that I've decided to forego upgrading any laptops as all of the ones I'm responsible for are over 3 years old and most of the unsuccessful upgrade complaints I've heard of came from laptop owners.

Any experience or thoughts? Perhaps I should take this as a sign that I should keep this machine on Win 7?
